Step-by-Step Descriptions of IVF Procedures
These include in vitro fertilization (IVF), which provides for traditional insemination, intracytoplasmic sperm injection (ICSI), assisted hatching, embryo cryopreservation, blastocyst culture, TESE/MESA male factor testing as well as embryo biopsy for preimplantation genetic diagnosis.
Step 1: Control Ovarian Hyperstimulation
An Antagonist protocol, in which gonadotropin hormone secretion is suppressed to prevent premature ovulation, is often the first step. Once optimal suppression has been reached, daily injections of gonadotropins may be administered daily to recruit multiple follicles using ultrasound imaging and hormone assessments as markers of their development once these lead follicles go an adequate size for egg maturation via HCG administration before scheduled egg retrieval between 34 and 36 hours post-injection.
Step 2: Egg Retrieval
In a surgical suite, under intravenous sedation, eggs are removed. A transvaginal ultrasound-guided needle is used to aspirate follicles. At the same time, an embryologist scans fluid from these aspirated follicles to locate all available eggs before culturing them further before insemination takes place.
If sperm parameters are normal, approximately 50,000 to 100,000 motile sperm are transferred into a dish holding eggs for standard insemination.
Step 3:
ICSI fertilizes mature eggs when their sperm parameters are abnormal. It is performed under a high-powered microscope by embryologists with fine glass needles, which pick up a single spermatozoon for injection directly into egg cytoplasms. If there are insufficient sperm in semen samples (low count and motility, poor morphology, or poor progression), surgical retrieval may be used. ICSI must always be used to fertilize these newly acquired sperm.
Fertilization should be evaluated 16 – 18 hours post-insemination or ICSI. Fertilized eggs (zygotes) will be cultured in an ideal medium that supports their development before being assessed two and three days later; should sufficient numbers of embryos exhibit healthy development, these may be chosen for further growth to blastocyst stage cultivation using the specially tailored medium.
Blastocyst culture provides numerous advantages. At this stage, embryos have a higher implantation potential; therefore, fewer embryos should be transferred on day 5 to reduce multiple pregnancies and limit considerable pregnancy risk. Low numbers and poor embryo quality significantly reduce the chances of successful blastocyst development; for such cycles involving low numbers/quality transfers, it is advisable to transfer on day three for optimal blastocyst development, and embryo transfer should occur on day three instead.
Step 4: Assess Embryo Quality
Various criteria are used to judge embryo quality, assessing your source(s) essential in selecting appropriate ones for embryo transfer. On your transfer day, an embryologist evaluates and photographs each root early morning based on the rate of development and appearance of seeds before consulting your physician regarding which and how many sources should be transferred.
Embryos are typically transferred at either the cleavage (three days post oocyte retrieval) or blastocyst stage (day five) stages, depending on their quality and progress in the lab. A grading system is utilized in this regard.
Step 5: Embryo Transfer
Typically, the embryo transfer technique of IVF Cost in Pakistan takes place on day three when the embryos are in the cleavage stage (6–8 cells) or on day five when they have developed into blastocysts. The simple and painless procedure of embryo transfer involves loading the embryos onto soft catheters and inserting them via the cervix with the aid of ultrasonography into the uterine cavity through the vaginal wall.
